Registration Process For September

To register, every student must complete a REGISTRATION AND PROGRAM SELECTION FORM. As well, the student and parent/guardian, upon reading the school board's computer policy, must fill out a systems access form to receive a pin number for personal access to the school's computers.

All registrations will be confirmed once a completed and properly signed registration form is on file at the school office.

1. Non Feeder Schools: Students from other school boards:

Registration and program selections forms may be obtained by scheduling a registration appointment with an SJSH guidance counselor. (705)494-8600 x 3234.

Please contact Students Services to schedule an appointment for grade 8 non-feeder Registrations.

A parent or guardian must be in attendance and bring along:

  • Proof of Ontario residency (ie: driver's license, utility bill, etc.)
  • The last report card of the student (elementary) or a credit counseling summary
  • Results of Ontario Secondary School Literacy Test (if applicable)
  • Health Card Number
  • If you have been identified as exceptional, you must also bring: the most recent copy of (1) the I.E.P.
    (Individual Education Plan) from the grade 8 school, (2) the most recent copy of the I.P.R.C. (Identification Placement and Review Committee) form from the grade 8 school.
  • If not an Ontario resident, the proper forms from SJSH must be completed before registration can take place.

2. Feeder Schools: Students currently in grade 8 in our school board

All details will be handled through the principal of your current school and the SJSH student services department. Students will have the opportunity to visit the school during our grade 8 visit that is hosted every year for our potential students. On-Line Registration sessions will take place with the grade 8 students, teacher and SJSH Guidance Counselors during February and March. The most recent copies of your I.E. P. (Individual Education Plan) and I.P.R.C. (Individual Placement Review Committee) forms must be attached to your registration form, if you have been identified as exceptional. The SJSH guidance counselors will return to the feeder schools in early March if parents want to set up individual meetings. Notes will be sent home with students to set up appointments.

How Grade 8's or New Students to North Bay Register

  • Call in May, June, or August and speak to the secretary of Student Services to set up an appointment with a guidance councilor. (705)494-8600 x 3234.
  • To expedite this process, have a credit counseling (grade 9-12) or report card for grade 8s forwarded to student services, as well as all additional documentation listed above.

How High School Students Can Transfer to SJSH

  • Call in June or August and speak to the secretary of Student Services to set up an appointment with a guidance councilor.
