2025-2026 School Year Registration Process (Current)
To register for the 2025-2026 school year, every student must complete a registration and course selection form (see below). Completed registration forms should be emailed to SJSH Registration or dropped off to Student Services.

2026-2027 School Year Registration Process (Next Year)
To register for the 2026-2027 school year, every student must complete a registration and course selection form (see below). Completed registration forms should be emailed to SJSH Registration or dropped off to Student Services. Please refer to the below check list to ensure that all required items are submitted. Important Note: Additional forms are required for those that are born outside of Canada and/or are interested in the SJSH Hockey Skills Academy. Those forms can be found in the below checklist.

REGISTRATION PACKAGE CHECKLIST:
Note: Please ensure that the below items are included when submitting your
registration package ([email protected]). All registrations will be confirmed once completed.
- REGISTRATION FORM (Complete with signatures, contact details, course selections…)
- BIRTH CERTIFICATE (Copy is sufficient)
- PROOF OF RESIDENCY (Utility bill with address)
- INDIVIDUAL EDUCATION PLAN (Copy is sufficient, if student has an IEP)
- TRANSPORTATION FORM (Complete with signatures, contact details, OEN…)
- APPLICATION FOR DIRECTION OF SCHOOL SUPPORT
- GUIDELINES FOR STUDENT RESPONSIBLE USE OF TECHNOLOGY (Complete with signatures, dates…)
- OPT-OUT OF E-LEARNING GRADUATION REQUIREMENT FORM
GRADE 9 STUDENT REGISTRATION REQUIREMENT:
- OLD REPORT CARD (OEN should be visible)
GRADE 10, 11 & 12 STUDENT REGISTRATION REQUIREMENT:
- CREDIT SUMMARY (May be attained by previous high school, includes OEN)
ALL STUDENTS BORN OUTSIDE OF CANADA REQUIREMENT:
- STUDENT INFORMATION AND ELIGIBILITY ATTESTATION FORM (Document may be attained in below link)
